Internet Access Permission and Release

Dear Parent,

The New York City Department of Education has established rules and guidelines for student access to the Internet in order to help ensure that schools provide a safe online environment for your child. We have attached a copy of the “Internet Acceptable Use Policy,” and the “Superintendency’s Internet Acceptable Use Policy.” Please read these policies carefully and discuss them with your child. Students accessing the Internet through school must agree to observe these and any other policies and/or guidelines set forth by the Department and your child’s school. The Department reserves the right to restrict, supervise, and review each student’s use of the Internet to make sure that it is in compliance with these rules of safety. In an effort to promote responsible use of the Internet at school, The Department requires that students and their parents or guardians sign this Internet Access Permission and Release in order to gain access to the Internet through school. Should you have any questions please contact the school.

2. I understand that violation of any of these rules will result in the loss of Internet privileges for my child and may include disciplinary action. __________
3. I understand that the Internet is a powerful and important learning tool that can make information and interactivity available that I may not deem appropriate for my child. A filter may be used to block inappropriate information and The Department cannot guarantee that it will prevent access to all objectionable content. __________
4. I understand and acknowledge that The Department intends to use its best efforts to enforce the rules and policies provided to me, but that it is not possible for The Department to fully monitor or control all information and interactivity that my child may encounter through the Internet. __________
5. I Hereby give permission to The Department, my child’s school and its teachers to provide my child with access to the Internet. __________
6. I Hereby agree to release the Department, my child’s school and its teachers from and against any and all claims arising from my child’s use of the Internet at school. __________

IT IS EXTREMELY IMPORTANT THAT YOUR CHILD IS INSTRUCTED TO ADHERE
TO THE BOARD’S RULES AND POLICIES, AND WE URGE YOU TO DISCUSS THESE
RULES AND POLICIES WITH YOUR CHILD PRIOR TO RETURNING THIS
PERMISSION AND RELEASE.
